Abstract. There is certain evidence that extralevator abdomino-perineal resection can improve oncological outcome in treatment of low rectal cancer compared to conventional abdomino-perineal resection. Extralevator resection requires additional perineal reconstruction to close a vast perineal defect. We report the analysis of 67 extralevator abdomino-perineal resections, with different approaches in the closure of perineal wound repair used. 25 primary wound closures, 19 mesh repairs and 33 muscular flap reconstructions were performed. Perineal wound complications were observed in 61% after primary wound closure, 29% in alloplastic group and in 29% after myoplasty. The conclusion was made, that mesh and muscular flap reconstruction are superior to primary wound closure in short-term outcome.
