INTRAOPERATIVE RADIATION THERAPYAN INNOVATIVE TECHNOLOGY IN THE MULTIMODAL TREATMENT OF TUMORS OF THE GASTROINTESTINAL TRACT

Abstract. The paper presents a method of intraoperative radiation therapy (IORT), including theoretical, methodological and technical aspects, analyzed the results of the multimodal treatment of tumors of the gastrointestinal tract. It is shown that intensive courses of preoperative irradiation and IORT dose of 20 Gy are compatible with any surgical interventions on the stomach and colon, including extended and combined ones. Analysis of long-term results showed that combination therapy is more effective than surgery in the case of locally advanced forms, and when the tumor has the structure of poorly differentiated, signet ring cell and undifferentiated cancers. After the timing observations of patients over 15 years, no effect of high single doses of radiation on the development of metachronous tumors was registered. Future perspectives for use of IORT associates with the improvement of technical equipment including the use of mobile accelerators.
